Mask bad channels in ccd=33

      Description

      Furusawa-san writes:

      I found that some bias and dark frames for recent ccd=33 data failed
      to create due to bad readout chennels. I propose to add a defect region
      for two whole channels of ccd=33 in and after 2016-11 run for the next
      production.

                hsc: add bad amps on ccd=33
                
                One amp went bad on 2016-11-22, and the one next to it was also
                affected on 2016-12-23.
                
                The amp in question is the right-most in the raw data, but it is
                flipped when read in by the butler, so the masking is on the left.
