LDM-294 update to remove duplicate text, clarify 2C.08 CAM

      Description

      The dev guide on RFC was updated and contains an updated version of LDM-294 appendix C. This text should be removed from LDM-294 which should rather refer to the dev_guide. The dev guide also refers back to the same appendix - these refs should be removed.

      I have also changed ownership of network elements to C. Silva  and explained the relationship to DMLT.

            I think the original motivation for having this in LDM-294 was to enshrine this as official, change-controlled policy. This could presumably be replicated by having a note to that effect at the top of the Dev Guide page.

            The dev_guide is well controlled and more visible. I am happy to add a statement if that makes it clearer.
